referenceRequired- The application reference, as Kamma sent it to you.
postcodeRequired- The postcode of the property the application is for.
categoryRequired- One of the category values below.
filenameRequired- The name of the file inside the ZIP.
descriptionOptional- Up to 255 characters, shown to Kamma staff.
has_gas_supplyOptional- yes or no. Decides whether we need Gas Safety Certificate.
is_limited_companyOptional- yes or no. Decides whether we need Licence Holder Photo ID.
If a reference appears on more than one row, its gas supply and limited company answers must match.
Values for the category column 31
Case is ignored. Anything that doesn't fit goes under misc_additional.
